The game is built around sustained dread: players are hunted through dark levels by entities such as smilers, skin stealers and hounds, with jump scares and the constant possibility of death.
Hostile entities chase and kill players, and players can fend some off, but harm is not depicted with blood, wounds, or gore.
The premise centres on being trapped alone in an endless liminal space, with a draining sanity meter that brings on hallucinations and death.
Escape the Backrooms is a first-person cooperative horror game for one to four players set in the 'Backrooms,' a fictional maze of empty fluorescent-lit rooms and other unsettling liminal spaces. Players explore more than thirty levels searching for the exit while managing a sanity meter with food items; low sanity brings on hallucinations and, at zero, death. Hostile creatures — smilers hiding in the dark, skin-stealers that mimic other players, and dog-like beasts with human faces — stalk and chase the group, and most are meant to be avoided or fended off rather than fought head-on. Dread is close to constant, but there is little graphic injury: harm is conveyed through pursuit, capture and death rather than blood or gore. Online play uses in-game voice chat, and there are no in-app purchases.
The game frames its content as atmospheric survival horror. The danger is environmental and creature-based, and progress is measured by escaping and keeping the group alive rather than by killing; the violence that exists is framed as fleeing or warding off monsters, and the sanity mechanic frames the central tension as psychological endurance in an inescapable space.
