The plot follows Vito Scaletta, a returning veteran of the war, who climbs the ranks of a criminal syndicate to clear debts left behind by his father. Players steer era-accurate automobiles, operate illegal rackets, and take on syndicate assignments featuring lifelike shootouts, fistfights, and killings of human opponents, complete with blood. Coarse language occurs often, and the title carries mature sexual suggestion, a sequence at a strip club with bare-chested performers, and gatherable Playboy issues that display complete nudity. On-screen figures regularly consume liquor and tobacco. Unlawful acts—theft of vehicles, holdups, coercion for money, and killing, mostly aimed at competing criminals—form the backbone of the plot. Everything unfolds without an internet connection, and there is no multiplayer contact or spending.
