The player punches, kicks, stabs, slices and bludgeons voxel NPCs with almost any object, producing heavy stylised blood that coats characters and the environment.
The player's attacks break the voxel enemies apart as the core mechanic — caving in heads and cutting bodies into pieces with weapons.
Bars and discos are recurring settings and beer bottles are picked up as improvised weapons; alcohol appears as set dressing and items rather than being consumed for effect.
Paint the Town Red is a first-person melee combat game built from voxels, in which the player enters bars, discos and other themed arenas holding roughly eighty enemy characters and fights until they are the last one standing. Almost any object in the environment — bottles, ashtrays, chairs, blades — can be picked up and swung, and striking an enemy dynamically caves in, breaks apart or dismembers the block-built body wherever it is hit. Blood is heavily stylized and cartoonish but abundant, coating enemies and the surroundings by the end of a fight. Modes include sandbox scenarios with adjustable modifiers, a challenge arena, a roguelite campaign called Beneath with selectable fantasy classes and upgrades, and a level editor whose custom stages are shared through Steam Workshop. Online co-op play with voice and text chat is available. There is no sexual content, and the game sells no in-app purchases.
The violence is framed as chaotic, over-the-top slapstick rather than realistic harm — the stylized voxel gore and the goal of 'painting the town red' are played for comic spectacle, with no narrative weight given to the killing and no distress framing around it.
