What it is

This top-down roguelike shooter casts players as Isaac, a small child who escapes into a horrifying cellar when his mother—convinced she is carrying out God's command—attempts to end his life. Fighting is done by launching tears at swarms of hideous creatures, and the game holds hundreds of strange pickups that reshape Isaac's anatomy and his abilities. Its levels are procedurally built and differ on every attempt; the moment Isaac dies, the whole playthrough resets from the beginning. Visually the title is drenched in blood, viscera, and grotesque distortions of the body, with much of its artwork drawing on demonic and occult motifs—pentagrams, fiends, spirit boards, tarot decks, and pacts with Satan that are paid for in hit points. The themes it handles are dark, touching on the mistreatment of a child, faith pushed to fanatical extremes, and heavy hints that the boy dies or takes his own life. Chance-based pills and substances resembling drugs form a central system, while slot machines and fortune-telling devices stand in for gambling. Players can team up online with invited friends, though no built-in voice or typed messaging is included, and add-ons released later permit content authored by players.

How it's framed

The brutality and faith-driven horror arrive wrapped in a grim, blackly humorous tone instead of approval; Isaac stands as the wronged party, and the zealot-fueled aggression aimed at him reads as frightening and sorrowful. Devilish and mystical elements are stitched into the gameplay as alluring yet hazardous deals, and both the physical and mental anguish come across as unsettling rather than celebrated.
